• To use the Kawasaki City Online Pre-Application Service (hereinafter referred to as the "Service"), you will need a smartphone or computer with Internet access. 
After reading the Terms of Use and entering your email address, check the box next to "I agree", and then click the "Confirm" button. 
A URL for inputting your information will be sent to your e-mail address. Please verify your settings to ensure that you are able to receive e-mails from the "sumado.jp" domain. 
The URL for input is valid for 24 hours. If you are not able to access the site while the URL is valid, please enter your email address again from this page. 
If you enter the same e-mail address repeatedly to start using the service, a lock may be placed on your access. 

• After you enter your application information, you will be asked to enter your passcode to confirm your entry. 
The passcode will be sent to you via SMS (short message) or voice message. 
Note that you will not be able to complete your pre-application if you are unable to receive messages or voice messages. 

• Procedures cannot be completed online through this service. 
After submitting your application in advance, a QR code will be displayed. Please visit the ward or branch office and present the QR code at the service window to complete the procedure. If a proxy comes to the service window, a proxy form is required. 

• This service provides information on the expected congestion at ward and branch office service windows. 
Please refer to the congestion forecast information and check which day and time are convenient for your visit. 
Please note that this is not a service for making appointments for the service window, so you do not need to contact us to change or cancel your appointment, even if you are coming on a different day or time than the one you entered. 

• You can temporarily save what you have entered and resume later. 
However, depending on your smartphone or computer, you may not be able to temporarily save or resume your data. 

• Please submit a notification of any change of address within 14 days of your move. 
If you have a My Number Card and would like to apply for an updated card after moving, please come to the office on Monday through Friday, as the procedure cannot be done on Saturday. 

• The information entered in this service will be used to print out application forms and other documents when you visit the office as a notification of a change in address. The information you submit will be kept by Kawasaki City as a notification of a change in address.
